EFFECTIVENESS OF L-CARNITINE IN THE COMPLEX TREATMENT OF PREMATURE NEWBORNS WITH VENTILATOR-ASSOCIATED PNEUMONIA

Summary.
100 preterm infants with body weight at birth from 1540 to 2500 g and gestational age from 30 to 37 weeks were surveyed. 80 infants with ventilator-associated pneumonia received complex antibacterial, immunocorrective and syndromic treatment, oxygen therapy. 40 infants (group 2) were on the indicated basic therapy, 40 infants (group 1) received an additional a drag of L-carnitine at a dose of 100 mg/kg per day in two doses during 25 days enteral. 20 healthy preterm infants made up the control group. L-carnitine had a positive therapeutic effect: respiratory disorders disappeared faster, also motor activity, muscle tone, physiological reflexes were improved, but the duration of basic therapy and hospital stay were reduced. Biochemical parameters normalized faster (a decrease in the high level of lactic and pyruvic acids, the fall of lactate/pyruvate in the blood serum). The levels of carnitine and acylcarnitines in the blood corresponded to the norm in the recovery period. After the treatment the activity of bio-energetic enzymes of blood lymphocytes in the main group reached the norm, and in the comparison group increased, that indicated the voltage of bio-energy processes in the cell

TREATMENT AND PREVENTION OF ACUTE RESPIRATORY VIRAL INFECTIONS IN CHILDREN

Summary.
The article presents the epidemiology of respiratory viral infections (ARVI) in children, describes the issues of their differential diagnosis. General principles of treatment of respiratory infections, indications for hospitalization of patients are highlighted. The mechanism of action of an early inducer of interferon types 1 and 2 - Cycloferon® is presented. Its interferon-inducing, immunotropic, and antiviral effects are described. The effect of Cycloferon® on cytokine production is characterized. The pharmacokinetics of the drug, its tolerability, safety, dosage regimens, and the possibility of using it in conjunction with other drugs are described. The role of Cycloferon® in increasing non-specific resistance of the body is described, as well as the possibility of its use for emergency prevention of acute respiratory viral infections.

Summary.
Review in order to once again draw the attention of pediatricians, especially those engaged in the treatment of chronic hereditary and acquired diseases in children, to the problem of chronic stress, adaptation syndrome, complicating the course of the underlying disease, reducing the effectiveness of medical treatment. Attention is drawn to the main groups of researchers-pediatricians, including: E.V. Neudakhina with co-authors, K.I. Grigorieva with co-authors, as well as biochemists -O.V. Kosmachevskaya with co-authors. The researchers state: chronic stress, adaptation syndrome are most often characterized by a decrease in bioenergetics, autonomic dystonia syndrome, dysfunction of the gastrointestinal tract, especially in children with hereditary connective tissue dysplasia, heart malformations, allergic diseases, glomerulonephritis, pyelonephritis. Among the external factors of chronic stress and adaptation syndrome mentioned ecopathology, psycho-emotional effects, heat, cold effects, weather dependence. We offer medical and non-medical means of prevention and correction of chronic stress
